Dr. Mahmoud Shalaby

Dr. Mahmoud Shalaby

Co-Founder & Board Member

Dr. Shalaby is CEO and Co-founder at PT of The City and a licensed Physical therapist with over a decade of experience in the field. Also he's the Co-founder of City Care Partners. He brings firsthand understanding of the challenges and opportunities facing independent primary care practices in value-based care, and has been instrumental in building the provider network that forms the foundation of the organization.

Faris Hamtini, Esq.

Chief Compliance Officer

As Chief Compliance Officer and board member, Faris Hamtini ensures that City Care Partners meets all federal and state regulatory requirements governing ACOs. An attorney by training, he oversees the compliance program, risk management, and governance processes, and serves as the organization's point of contact for CMS audits and reporting.

Leon oversees all financial, strategic, and operational functions of City Care Partners. He leads the organization's participation in the CMS LEAD Model, manages payer and partner relationships, and drives the financial planning that supports sustainable, long-term growth.

Chief Medical Director

Dr. Byrne leads the clinical direction of City Care Partners, overseeing care management protocols, quality measure strategy, and clinical governance. She works closely with participating providers to ensure that evidence-based practices are integrated into care coordination workflows and that clinical outcomes drive organizational priorities.

Chief Compliance Officer

Faris leads the organization's compliance program, ensuring adherence to CMS LEAD Model requirements, HIPAA, anti-kickback statutes, and all applicable federal and state regulations. He manages the compliance hotline, conducts training, and serves as the primary contact for CMS audits and investigations.

Mahmoud manages the operational infrastructure of the ACO, including provider onboarding and credentialing, data integration, care coordination logistics, and day-to-day performance tracking. He serves as the primary operational point of contact for participating practices.

Director, Provider Recruitment

Donna leads the recruitment and onboarding of primary care practices into the City Care Partners network. She works directly with practice leaders to demonstrate the value of ACO participation, guide them through the enrollment process, and ensure a smooth transition into value-based care.

Mohamed Nabil Abdelnabi

Head of Quality & Provider Performance

Mohamed oversees quality measure reporting, performance analytics, and provider scorecards. He works with practices to identify care gaps, implement improvement initiatives, and ensure that City Care Partners meets or exceeds all CMS quality benchmarks under the LEAD Model.

Compliance Committee

Oversees the compliance program, reviews audit findings, monitors regulatory developments, and ensures the organization adheres to all CMS LEAD Model requirements and applicable healthcare laws.

Finance & Audit Committee

Reviews financial statements, oversees the audit process, monitors shared savings calculations and distributions, and ensures fiscal accountability and transparency.

Quality Committee

Monitors quality measure performance, reviews clinical outcomes data, and oversees strategies to improve care quality and patient experience across the ACO network.

Beneficiary Engagement Committee

Ensures that the needs and perspectives of Medicare beneficiaries are represented in organizational decisions, reviews beneficiary communications, and oversees engagement and satisfaction initiatives.

Network & Credentialing Committee

Oversees provider recruitment, credentialing, and network management, ensuring that participating providers meet all clinical, operational, and regulatory requirements.

Operations Committee

Reviews operational performance, care coordination workflows, technology infrastructure, and day-to-day execution of the ACO's strategic priorities.
